Congratulations on moving forward with Rooted Health Institute!

If you have received a conditional acceptance from RHI, please use this page to complete the required enrollment forms and documentation for your healthcare training program.

Some forms must be completed by a parent or legal guardian if the student is under 18. Please complete all forms requested by the RHI Admissions Team.

Important: Receiving access to this page does not mean that all enrollment requirements have been completed. Students are considered cleared to begin only after RHI has reviewed and approved all required documentation.

Your Enrollment Steps

Complete Your Forms

Complete all required student and parent/legal guardian forms.

Submit Required Documents

Provide identification, health/immunization records, and other requested documentation.

Complete Clearances

Complete applicable screening, drug screening, BLS/CPR, and program-specific requirements.

Get Cleared to Start

RHI Admissions will review your file and notify you when you are officially cleared to begin.

Provides parent/legal guardian authorization for participation in RHI healthcare training and applicable educational activities.

Communication Authorization

IF APPLICABLE

Allows RHI to communicate with the student's designated school counselor or authorized school representative regarding enrollment, progress, scheduling, funding, and program coordination.

Media Release

OPTIONAL AUTHORIZATION

Allows the student and parent/legal guardian to choose whether RHI may use identifiable photographs, videos, audio recordings, or other approved media for website, social media, promotional, educational, and outreach purposes.

Externship Availability

REQUIRED IF APPLICABLE

Students tell RHI when they are available for experiential training, including summer break, winter break, other school breaks, or during the academic year when it will not interfere with school, athletics, or other activities.

Participation in promotional media is optional. Declining media authorization will not affect a student's admission, enrollment, academic standing, instruction, or participation in the program.

▣   Required Documents & Clearances

Students may be required to provide or complete:

  • Government-issued or other acceptable identification
  • Health/immunization records, when applicable
  • TB or other health clearance, when applicable
  • Background check, when applicable
  • Drug screening, when applicable
  • Current BLS/CPR certification, when applicable
  • Program-specific documentation
  • Externship/clinical requirements, when applicable
  • Additional documentation requested by RHI or an experiential training site

♥   Already Have BLS/CPR?

Students who already hold an accepted, current BLS/CPR certification may submit documentation to RHI for review. Students who do not currently meet the requirement may participate in an RHI-arranged BLS/CPR course when available.

Testing Accommodations

Have an IEP, Section 504 Plan, or documented accommodation?

If you indicated an accommodation need during your application, RHI Admissions will provide instructions for securely submitting appropriate documentation.

Documentation should be submitted as early as possible so RHI can support your training needs and, when applicable, assist with requests for accommodations through national certification or testing organizations.

National certification and testing organizations establish their own documentation requirements and make final decisions regarding examination accommodations.

🎓   Program-Specific Information

Pharmacy Technician → Indiana Board of Pharmacy requirements apply. CCMA (Clinical Medical Assistant) → Review program and national certification requirements. Phlebotomy Technician → Review program and national certification requirements. Medical Billing & Coding → Review program and national certification requirements.

What Happens After I Submit Everything?

RHI Admissions will review the student's enrollment file. If additional information or documentation is needed, we will contact the student and/or parent/legal guardian.

Once all applicable enrollment requirements have been completed and approved, the student will receive confirmation that they are Cleared to Start, along with orientation and program-start information.
